“Are you certain?” I asked, maintaining eye contact. “Because the logs show a slight increase over the past few weeks, consistently. And there’s no updated order signed by me or his primary pediatrician.”

Her brow furrowed deeper, her hands stilling around her bag. “An increase? But… Dr. Kincaid, he updated the standing orders during his last consultation on Leo’s sedation protocols. He said it was a temporary measure to help with the night terrors and prevent further seizures.”

A cold certainty settled over me. Julian hadn’t just altered the logs; he’d created a fraudulent verbal order, relying on Sarah’s trust in him as a senior surgeon. He had used his authority to manipulate her, making her believe she was following a legitimate medical directive.

“Dr. Kincaid?” I repeated, my voice betraying no emotion. “Sarah, I haven’t authorized any change to Leo’s sedative protocol. And all such changes require a signed order from me or another pediatric neurologist, not an orthopedic surgeon, even one of Julian’s standing.”

Her face went pale, a slow dawning horror spreading across it. Her eyes darted around the empty lounge, then back to mine. “But… he assured me. He said it was critical for his neurological stability.”

“He lied,” I stated flatly. “And your signature is on every one of those logs.”

Her hands flew to her mouth. She looked like she might be sick. “Oh God. I… I had no idea. He seemed so concerned, so professional.”

“I know,” I said, my voice softening just a fraction. “He preys on trust. But Sarah, this is a serious breach. Falsified medication records. Administering an unauthorized dose to a child with a complex neurological condition.”

She stared at me, her chest heaving. “What do I do? My career… I followed orders.”

“Orders that were never legitimate,” I reminded her gently. “Julian set you up. To cover his own tracks.”

I paused, letting her absorb the full weight of it. “Do you have access to the complete, hardcopy night-log binder for Leo’s care? The one Julian usually keeps in his office, or locked in the medical cabinet?”

Her eyes widened, a flicker of understanding, then resolve, hardening them. “He kept it locked in his office, yes. Said it contained ‘sensitive family information’.”

“Can you get it?” I asked, my voice low and urgent. “And give it to me, without him knowing?”

She nodded slowly, a single tear tracing a path down her cheek. “Yes. I have a key for unit access. I’ll get it tonight.”

“Thank you, Sarah,” I said, the words heavy with meaning. “You’re doing the right thing. For Leo, and for yourself.”

She didn’t speak, just turned and walked quickly out of the lounge, her posture stiff. Hours later, a small, black, leather-bound binder appeared on my desk, placed discreetly beneath a stack of incoming mail. It was unlocked. My heart pounded as I opened it.
